How to Obtain Bleaching Gel Refill Syringes

Steps:

1. Determine how many syringes to give (the code for one syringe is D9972-1 and the code for four syringes is D9972-4).
2. Add to treatment plan.
3. In forms, add "SDM Tooth Care Products Form".
4. Select 10% or 15% carbamide peroxide.
5. Have form approved by faculty.
6. "Running man" form to finance group.
7. Walk patient to finance window, where they will pay for refill.
8. Have finance window "running man" to dispensary.
9. Pick up syringe(s) at dispensary and give to patient.
